Dr. Uddin is a psychiatrist certified by the American Board of Psychiatry and Neurology. He is not an addiction or forensic psychiatrist. The evaluation includes:

· review all available records, including but not limited to academic records, outpatient psychiatry, hospitalizations, psychotherapy, social work, neuropsychology, and all other pertinent mental health treatment. It is important to provide the most extensive records available to provide the best assessment.

· A comprehensive clinical interview obtaining detailed history including psychosocial or developmental problems; academic and employment performance; legal issues; substance use/abuse (including treatment and recovery); aviation background and experience; medical conditions, and all medications used. He conducts a mental status exam, which provides behavioral observations during the interview.

· An integrated summary of findings with diagnosis(es), opinion, and recommendation(s) for treatment, medication, therapy, counseling, rehabilitation, or monitoring should be explicitly stated. $400 deposit required.
